Maybe it was just me, but for the longest time I could not create a toc using Mobipocket, first I didn't know what information to add and second I kept getting an javascript error message. Both things were resolved in this blogpost:

It gives step by step instructions and in the comments section I discovered how to get rid of the error message. All I had to do was to go back to IE8, for some reason IE9 screws up Mobipocket.
I've used this method on the last 4 eBooks I've formatted and it looks great, finally Kindle Previewer recognizes the TOC.

It's a complex eBook and haven't gotten to the chapter with the tables yet. Will let you what I did.

I'm using html code to convert the book into an epub.

I do my writing and editing in Word and then strip out all of Word's extraneous code using Notepad ++. Then I add formatting (bold, paragraphs, etc) with the ..."

I do, I have version CS 5.5, although I only use it for POD books, so far don't like how the epubs look. Maybe I just haven't done it properly.

I just got my sample and I'm very impressed as well, will definitely buy it. It is always good to reinforce my skills and maybe learn a thing or two. Thanks Tim!

For .prc or .mobi I use the Smashwords style guide in MS Word, then do the conversion in Mobipocket.
For POD books I use InDesign.
Recently I used Kindle Previewer to convert an epub into mobi so we could view a file on Kindle Fire.
