My book has now appeared on Barnes and Noble but it is via Smashwords. I have no idea if I need to add a new version as I am not sure if it just keeps the SW ISBN and can be found via that?
I don't want to cause issues and add a book to be combined which isn't needed. Anyone have any clue on this?

If your nook/ebook for sell on Barnes and Noble has a bnid starting with 294# (id unique to bn and in proprietary bn pubit file type), then, yes. Create edition with 294# number in isbn13 field, leave isbn10 field blank, format= ebook, edition= nook edition.
In both cases (978# or 294#) the Barnes and Noble purchase link works.
Similar to isbn ebook guidelines for when you do/don't need additional isbn numbers—just for goodreads read "additional editions" instead of "additional isbns."
Exact same ebook (even if not downloaded in exact same file type) with exact same isbn = only need one edition no matter how/where sold or what devices used to read.
(Ditto for other publishers, authors or distributors of an ebook - one isbn 978#=one edition)

Author: Redden Sanders; new name: Willett Thomas
Former title: Low Time in Arcadia; new: Raised by Hand, Lifted by the Tides
Thanks!

A new edition is added with the details. The primary author name is the first published name. The books are also combined
Do you have a new ISBN or ASIN / cover

No. It's on Amazon.

We cannot take data from booksellers you need to provide it here and a link for the cover from a NON bookseller link

3. New synopsis of novel:
When we first meet Lily “Lil Bit” Dalton, she is quite literally drowning. This “lip flapping” event, along with others, takes place in the quirky, yet racially divided waterway township of Arcadia, Florida in 1954.
In Raised by Hand, Lil Bit recounts the years between ten and thirteen spent in the care of the woman she considers to be her grandmother, "Bubbah," and this woman's alcoholic son, Bobby. As the story opens, it’s been five years since Lil Bit’s mama, Violet, “dropped her off like nothin’ but a bit of dirty laundry,” when on “Ditch Thursday,” Lil Bit is pulled from the river by Sheriff Marin. The Sheriff’s act of heroism leaves her not only with the secret of what lies beneath the water, but also with gnawing doubts about Sheriff Marin, “the only white man worth a damn in all of Arcadia,” who also knows the river’s secret, but tells no one.
Like the tales told by the young heroines in the classics To Kill a Mockingbird and the Secret Lives of Bees, Lily “Lil Bit” Dalton’s quest to reunite with her mother, while also keeping the secret of the Massaquatta’s murky depths, takes readers on a tumultuous ride filled with adventure and mystery. But more importantly, Raised by Hand answers the age old question: What does it take to be a family?

The cover is the same. I am not sure if I add this is will muck things up as it is the same ISBN as Smashwords?
This edition you already have links to Kobo. It does use the same ISBN as Smashwords:

I promise I'm not stalking you :)

We don't add platform-specific eBooks unless they have a unique ISBN and/or identifier (ASIN, 294 BNID, Etc).
So, if someone published a book and bought a separated ISBN for B&N, Kobo and a PDF version - we would add all three. If one ISBN is distributed by B&N, Kobo, GooglePlay and iTunes, only one is added unless they are given a seller specific ID.

I have the B and N and the paperback and Kindle well so I think I am sorted now. Thanks for the advice.

Instead of going to the book's page on the Kobo site it goes to the homepage and puts a little lightboxy image in the corner that I can hover/click and buy from.
Here's a screenshot of what I'm talking about:

I've tried several books with the Kobo link and all have had the same result. This looks like it might be a recent change on Kobo's part (I'm pretty sure it wasn't happening last time I checked Kobo links, which wasn't too long ago). It may be something with the script on that box that's causing your crash?
I have now published this as an e-book in another market than Amazon, as well as the Kindle version. I can now add this again and asked for them to be merged? I have a new ISBN for this version.
ISBN 9781301664696
everything else is the same.
I am happy to do the necessary if this is the case but I don't want to make more work for the librarians if it is incorrect.
