can anyone tell me the name of this book? I've read in the last yr and its been driving me crazy for 2 days.

Its a book about a city that has had a wall built around it. they have better medicine and atmosphere and belive noone can live one the outside. A boy accidentally goes out of the wall when running from a teen in the subway tunnels who is trying to kill him because the boy saw the teen and his girlfriend having sex in a park. this is illegal because of stds and pregnancy. the population has to stay down because of room inside the wall. once outside the people who live there stitch up the boys injuries and send him back. noone believes him about the people and say he was confused because of his head injury. after the boy graduates high school he goes back through the hole to prove to himself it happened.he comes back with a girl he falls in love with but is chased back out because it was illegal to bring her in. he ends up staying out there and getting married and having children. the story ends up with his mother coming out to visit his grave
