What's the Name of That Book??? discussion

I read this book awhile back and forgot the title & the author.

The heroine of this book is a high school grad (i think) and just had a heart transplant. She went to buy an urn for the ashes of her (old) heart and wants to scatter her old heart's ashes, while she's going to travel from one place to another. The hero saw her purchase the urn and decided to travel to with her. While traveling together the girl have some emergency problems of her heart (i guess) and they need to go home. The guy is having this guilt that because of him, she's having those problems/complications. They went to college separately, the guy surprises the girl at her college and they LHEA.

Would really appreciate it if anybody can help me remember the author or title of the book. I cannot remember the name of the characters, but that is story all about. Thank You!
