What's the Name of That Book??? discussion

Okay so I'm pretty sure this book was a later book in a series. In it, the group of characters have to travel, either back in time or to another dimension, to fix something that has happened or to chase someone or something. This group includes a young girl and a character who is trapped in the form of a teddy bear or some other toy or tiny creature.

My brain tells me it either has "sword" somewhere in the title of the series or contains a magic sword or something. I read it at some point in the nineties, I think, so it could be from there or earlier.

A guess - The Woven Path (1995) by Robin Jarvis? It has different cover images. Time travel, talking teddy bear.
