Though we think the Lulu website charges too much for their print-on-demand services, we want the book to be available for purchase in the United States (and worldwide, of course). Therefore, we have made it available through Lulu as a PDF download, but not as an actual paperback for purchase (as we consider their paperback edition way too expensive).
