Yes, lots of possibilities. I think of Beatrice, even though there was nothing Miles could have done to save her, and he realizes that now. But it may be an experience he never talked with Cordelia about.
Yes. Sadly while Miles is a nice guy, he's done quite a few things that that would make just about anyone's hair curl.
