When constructing plot events, do you ask yourself why it has to happen this way and not any other possible way? Do we as readers have to suspend our disbelief to enjoy a story?

You can't plot and event and not wonder why it happens to happen one way and not another. All you can do is try to tell the story the best way possible. If that involves suspending your disbelief, then sure, yes, you have to do that.
