Usually, ideas come to me when I see something I really like or think has wasted potential-- side characters that could have been given more page-time or things to do, mentioned concepts and ideas that were never explored, and plots with missed opportunities.
My response to those is to write down what I would have done if this were my world/character/plotline and everything unfolds from there.
Inspiration to put together everything and start the story kicks in when I create the best character to accompany the idea. Without the right protagonist, the story can't go anywhere.
