Where did you get the idea for your most recent book?

Cameron Reilly The book started as an exploration into why so many organisations - business, political, religious, military, law enforcement, etc - seem to make horrible decisions that impact negatively on society. This lead me to think about the worst managers I have worked for and with over my career. And I realised they all had one thing in common.
