If you are going to kill someone you have to ask the other person first.
For example:

Player#1: John tightened his grip on Michelles throat. ((Permission to kill?))

Player#2: ((Permission))

Player#1: John stopped when he felt the body go limp.

OR if the person says no, then you can hurt the character but not kill them!

One question. Do you have to ask permission to mangle someone, like cut off their arm or something?

Mmm depends on how you are with the person you are rping with. We all don't ask each other every two seconds if we can stab the other, cause that's kinda expected. If you feel like it's a big thing, I would ask the.person beforehand just to be safe.
